People showing up that you didn't even invite
The guests that decide to bring their ex-cousin-in-law's nephews & nieces, sisters, grandma and baby daddy just because they're hungry. WTF?

This is where the hostesses and ushers come in...it is there job to ensure bottlenecks like those do not happen.
During the rehearsal dinner, inform them of specific things you want happening during your ceremony...
1. Escort late guests to seats
2. Escort them Only after they (usher) have scanned the room and procurred an available seat
3. Insist guests respect the inviolability of the bride & groom's union
